Changed your marital status? Need to add a dependent or change a beneficiary? Retired? If any of these events apply to you, please notify us as soon as possible so that we can continued to provide you with superior customer service.
To our providers - If you have a change of address, legal name or federal EIN, please send in a W-9 in order for us to update our system and prevent any delays.

Did you know...? Effective June 1, 2010, dependent children under the age of 26 are no longer required to provide proof that they are full-time students in order to be covered under a parent’s health insurance policy. This change was enacted by the Health Care Reform Bill (The Patient Protection and Affordable Care Act), and covers eligible children up to the first day of the month following their 26th birthday.
Previously, only unmarried dependent children who lived with a parent until their 19th birthday were covered unless they were determined to be full-time students, in which coverage was extended up to age 24. Now, in addition to extended coverage up to age 26 without proof of full-time student status, dependent children are no longer required to provide proof of residency or of support.
Coverage extends not just to children, but to foster children, adopted children (and children placed with you for adoption) and stepchildren for whom no other insurance coverage is provided by a divorce decree, acknowledgement, paternity court order, state order, or for whose health insurance a biological parent is not otherwise responsible.
